The Jack Miller Center for Peripheral Neuropathy Clinic is part of the University of Chicago Hospitals, an academic medical center located on the campus of the University of Chicago, in Hyde Park.

The Jack Miller Center Clinic is located on the 4th floor of the Duchossois Center for Advanced Medicine (DCAM).

Patients and visitors have the option of self-parking or using valet parking. An enclosed pedestrian sky bridge connects the parking garage to DCAM.

If this is your first visit, please go to Outpatient Services to register, before your appointment. Registration is available on the second floor for those entering through the parking garage and on the first floor for those entering from valet parking or the front doors. Representatives will confirm your mailing address and insurance information, and will help guide you to your appointment location.
